Step 1: Complete the Intake Form in English or Spanish.

Step 2: A member of our team will reach out to you to continue with the application process.

Step 3: Complete any forms you receive, or ask for assistance completing those forms.

Step 4: We will contact you. This can take time, please be patient as we work hard on your application. Please direct questions or concerns to your caseworker and they'll be happy to help you.
